  • Government-issued photo ID: Such as a valid passport or driver’s license.
  • Vaccination records: If available, bring your immunization records. Translation to English is preferred but not mandatory.
  • Form I-693: Fill out the first three pages at home but don’t sign the form until your appointment.
  • Medication list: If you are taking regular medications or being treated for chronic conditions.
  • Medical records: If applicable, bring written records of any treatments or hospitalizations for mental health, substance abuse, or psychiatric conditions.
  • Payment: Fees are required at the time of service and are not covered by insurance (cash, credit, and debit accepted).

What to Expect During the Exam

During your visit, the doctor will:

  1. Review your medical and social history and your immunization records.
  2. Conduct a physical examination.
  3. Administer required tests, including:
  • RPR Blood Test (Syphilis): For ages 18–44.
  • Neisseria Gonorrhea Urine Test: For ages 18–24.
  • QuantiFERON Blood Test (TB screening): Required for ages 2 and up. If positive, a chest X-ray will be needed.

Additional tests, such as a drug screen, may be required in certain cases.

What Happens After the Exam?

After completing the medical examination:

  • The doctor will certify Form I-693, including test results and vaccination history, and place it in a sealed envelope for USCIS. Do not open the envelope before submission.
  • You’ll receive a copy of the completed form for your records.

The examination results are valid for one year from the date of the exam.

5. What if my TB test is positive?

If your TB screening test is positive, the doctor will order a chest X-ray. If the X-ray shows no signs of active TB, the doctor can complete the form. If active TB is detected, further treatment is required before the form can be finalized.
